Prior to your first visit, please print out and complete the New Patient Forms and bring them with you to your appointment. These forms are also available in our office for your convenience.
Please remember to bring a copy of your dental insurance card, any x-rays and/or referral information provided by your referring doctor, and a list of current medications.
If you have a significant medical history or require antibiotic pre-medication prior to dental appointments, please notify us prior to arriving for your first appointment by calling 503-652-2615 during regular business hours.
